Register as a New Patient

We welcome all new patients at Coll Medical Practice.

Attached are the registration forms; please complete both. Please download and print and fill out. You are also able to get a hard copy from the reception desk. Once completed, please speak to the receptionist and bring the required photographic ID. If you are transferring from another practice in Scotland, we can usually get your records quickly but if you are coming from elsewhere in the UK, this may take longer. If you are coming from abroad, we will not automatically get copies of your records. It can be helpful for you to hand in any records you may have however if they are in another language, you will need to pay to get these translated (we can arrange for this to be done via the NHS translation service).

New Patient Medicals

We encourage you to have a new patient medical with the practice nurse when you join Coll Medical Practice. She will be able to review all of your records as well as making sure we look after all of your chronic illnesses correctly. She will arrange any appropriate follow up of tests or medications. This is particularly important if you have some chronic medical conditions, however is it not compulsory.

Temporary Patient Registrations

If you are only visiting or staying in Coll for a short period, you are able to register as a temporary resident if you need urgent medical care. Please speak to a member of the admin team. You will be required to fill out some paperwork before we are able to see you (this is so we can make an entry in your medical records of the consultation and also provide a prescription if required). If you are registered at another practice within Forth Valley, we encourage you to speak to your own practice rather than registering temporarily with us. You can be registered as a temporary resident for up to three months and then after that, you would become a permanent resident of Coll.

Non-English Speakers

Here are some leaflets in various languages explaining the role of the GP surgery. They have been produced by the NHS. They have been tested with user groups and use clear language.
